APPLICATION FORM

Documents to be enclosed with the Application Form:
- a biographical note (musical education and artistic achievements) in Polish or English
- a copy of an official document confirming the date of birth
- 2 recent photographs
- a record of payment of the 100 Euros registration fee

Applications and the enclosed documents should be sent by mail by 1st February 2010
